I decided to make a new thread because I just sorta wanted to get away from the old one. Although he still needs vibes and good wishes and thoughts, we are through the first crucial 24 hours and now we can focus on getting him better and his recovery.

Short version, we are dealing with Spontaneous Idiopathic Pneumothorax. Suddenly, for unknown reasons, Zander's right lung collapsed due to air that leaked from his lung into his chest cavity sometime Wednesday night. The emergency vet was able to remove 1500ml or 2 liters of air and kept him on oxygen until 6am. He was then transferred to our regular vet at 8am and we were cleared to take him home after xrays and bloodwork at noon. Xrays were clear but bloodwork showed his liver values were really high - 310. It's been about 24 hours since then, which is the major window to make it though. It means the hole in his lung has been sealed naturally and isn't continually leaking air into his chest cavity. So now we are on MAJOR bed rest. He can't run, get excited, or anything that would increase respiratory or heart rate. I have him doped up on Benedryl to help keep him sleepy and cooperative. Last night we had a minor scare due to him getting into the Denamarin and eating over half the bottle. It wasn't toxic, but I sorta freaked and induced vomiting anyway. The heaving doesn't seem to have done any damage.

Day One Update: Zane has been doing good. We're suffering from a bit of separation anxiety, which I honestly can't blame him for. He's pretty happy to take the Denamarin. I guess it is extra tasty since he felt he needed to eat ALL OF IT last night. His appetite is definitely not suffering. He is, however, very lethargic and possibly in quite a bit of pain. He is clumsy and his back end isn't completely with it. Today is better than it was last night, though. It is possible that it might just be his old man hips after all the fighting and restraining. He has been dreaming really vividly, he wasn't getting much sleep due to it which is another reason for the benedryl. We're doing nice and calm SLOW eating out of my hand so he doesn't get carried away. He starts a round of Amoxy tonight for warding off infection. Should have been yesterday, but we had issues with the Pharmacy. Respiratory rate has been normal, gums are nice and pink, no fever or increased heart rate. So far, things are looking good. If we can make it through the next two weeks, we're not necessarily in the clear, but we are out of the primary danger zone for the same area to blow out again. The xrays didn't show any lesions, so we don't know if there are others.

Here's some pictures from yesterday. There is a video on facebook from Wednesday night when he was starting to show signs of distress if anyone is interested in seeing it. (that I am thrilled I took - there is very little information on this anywhere on the internet.)







The emergency vets sorta murdered his front legs. They were attempting to get an IV in him and he apparently had NONE OF IT. Despite a collapsed lung, he put up one HELL of a fight between the xrays and IV. They doped him on Valium for the thoracocentesis(where they inserted the needle between his ribs to remove the air). We are going to wrap his legs to help with the razor burn. If anyone knows a gentle product that won't irritate his legs further to remove the matted blood, please let me know!

Poor guy! I am a big fan of sea salt soaks, and it might help his leg. Dissolve 1/4 tsp non-iodized sea salt (not regular table salt) in 1 cup of warm water. Soak a rag or cotton balls in it and hold it on for a while, then rinse with fresh water.

Really hope he continues to do well!

As for the blood on his fur, you can take some hydrogen peroxide, moisten a cotton ball or gauze pad, and rub the dried blood off. It will clean him and remove staining.

Aw, poor Z. I hope he continues to do well, I can't imagine how scary that was!

Unfortunately, it isn't a disease. It's just something that happened. Barely borderline a condition. Usually you see pneumothorax in dogs who sustained massive trauma, enough to damage the lungs and therefore leak air into the chest cavity. Even in spontaneous pneumothorax there is at least a cause like Valley Fever, lung cancer, heartworms, etc. Then there's idiopathic spontaneous pneumothorax....where there's just no apparent reason what so ever. Until he heals and he gets a CT, we won't know if it was truly idiopathic(just a freak thing) or if there are underlying lesions and/or cause...whether THAT is idiopathic or due to something like an infection, irritation, etc.

It sucks. I can't even sit here and blame myself for anything. I did everything right. Every vet we've seen thus far thinks he's a 2 year old, six years younger than he actually is. It wasn't caused by anything I did or any lack of care to his health. It was just freak. It just happened. And I can't even fix it or fight against it. :( I hate thinking about it. It's such a frustrating situation to be in.

Happy update time, though. :)

Day Two - We had a minor scare yesterday when he did a hack/cough right before we went out to go get his antibiotic from the pharmacy. Since his vet was out that way, we went ahead and took him out there for a re-check. She gave us more hope, saying his breathing and heart sounded good. They also (FINALLY) showed me both sets of his x-rays. The first set was scary. The second x-ray still showed a lot of air. His heart was still elevated. But it was tons better than the first one. She buried more into what may or may not have caused it and mentioned that the entire clinic was on his case. It's a rare thing, and they are studying every piece of information we can give them. They are willing to do whatever it takes, and if he showed anymore accumulating air in his lungs they would set him up in a chest tube. She also mentioned that it was possible the trauma could have elevated his liver values, but we would wait and see.

We're set for another set of x-rays and bloodwork on Wednesday or Thursday.

In the meantime, I bought a stethoscope to try and monitor him more closely. I can hear his heart better now than I did before(it was really muffled), and closer to the bottom of his ribcage. I hope that means some of the air is getting absorbed. His breathing was super clear. Gums are okay. His breathing is normal, if not a bit heavy at times. But not shallow, and only when he is lying on his side. There is a nasty bruise starting to form on his side. So I am thinking pain, probably. I'm going to call Monday and ask if there's anything we can give him. Refuses to take Benedryl and his antibiotics. And he is sadly too smart to just take it in a hot dog or something. He always eats around them. So they're being shoved in the back of his throat for now, with lots of treats and lovings afterward. He was a bit more active today. Enough to yell at me when I was leaving for work. But still lots of sleeping, lots of clumsy walking...just sorta dragging himself from one sleeping spot to another. He isn't even bothering to try and take off the socks I put over his shave spots to keep him from bothering the razor burn. We're using a bit of BagBalm for that.

Maybe if he promises to leave them alone we can set him up with cool looking bandannas or wristbands or something until the hair grows back a bit. :p

Day 3, 4, and 5 Update: Day 3 was a lot like Day 2. I started weaning him off of the Benedryl that night to see if there was any major difference in how he felt. He definitely perked up a lot. Trying to run up and down the stairs, a few zoomies, etc. Everything still looked and sounded more or less okay.

Day 4 I ended up having to dope him up on Benedryl again. He is feeling more or less normal. Enough to run around and zoomie and have tons of fun giving me heart attacks. :rolleyes: Best news on that day. I could hear his heart loud and clear on the bottom of his chest - indicating that either some of the air has dissipated or his heart is no longer elevated. Good news either way.

And Day 5, today, was the same. :) Lots of clear heart and breathing sounds, nothing is muffled or hard to find like before. X-Rays and bloodwork tomorrow to see where we are and how we stand. Crossed fingers!!!

He still hates the Benedryl and antibiotics and refuses to take either. So we've had to either shove it down his throat or hide it in super-super yummy things that he isn't going to question. Like chicken nuggets. :p

Day 6 Update: More good news today! Resp. rate & heart rate are normal, nice even improvement. Vet trip today was good. He is slowly accepting that this is just going to happen d is more or less happy to comply - in exchange for love & treats. X-rays looked better today. His heart is still mildly elevated compared to normal, but loads better than his previous x-rays. His first x-ray showed his heart crushed against his spine. Second x-ray showed his heart was barely more visible. So we have a LOT of improvement, but not quite normal. We are in again next week before we leave to check one more time before the move & transfer to a different vet. But I will really miss Dr. Ruth. The clinic has been SPECTACULAR. Blood work is next week as well. Might as well wait.

So happy to hear that he's improving! Hope he continues to just get better and better :)
