She sounds on the small size .... I'd check with the breeder or your vet . Been a long time , but it seems to me I gave 1/2 cup 3 X s at that age . She should be due for shots , so I'd say your vet should tell you . Take in a stool sample .

Do check with your vet. It sounds a little on the light side but I'm forgetful too about my puppies. I just have a gut feeling. I do recall that older pups eat considerably more than adults.

Showpug is right--you are really underfeeding. Why are you feeding a medium to large breed dog a food meant for small breeds? This could result in growth problems.
Wow. This is really dangerous. According to Eukanuba, if you were correctly feeding the large breed puppy food, you should be feeding her 3 and 1/3 cups per day. [B]Please switch to an appropriate food!/B] Have you considered switching to a better food?
