OVC tomorrow

Tomorrow is the big day for the final health test for Karma, she has passed every other breed specific one and now its her hips and elbows tomorrow.
I know OFA is far more desirable, but I have my reasons for choosing OVC. I am quite certain that when the time does come to figure out if I am or am not going to breed Karma I will insist on doing OFA to see how "good" her hips are since pass/fail may not cut it for me but I will deal with that when the time comes.

I'm so nervous, I don't know why but health tests make me nervous. Her CERF I swear I lost a good 2 days sleep over lol, as well as her thyroid ( that was a week of nail biting to get all results back)
I do have a point to this question though. I lost my little list of stuff I need to bring, I think I have it all but I would hate to drive 2 hours (plus construction) to find out I forgot something.
Can anyone help? What MUST I bring ( yes I hope to remember the dog)

Paperwork . Bring the paperwork required from OVC as well as your dogs registration papers. If she is microchipped, bring that info as well.
