We usually only scoop once a day.
No liners used. They end up ripping and making a big mess. I would say it gets new every month or so. Usually when it starts getting low. Umm never have cleaned the box its self. Shes a pretty good about pooping in the middle. If she doesn't then the daughter cleans the side up.
